1. What the Service is

The Service provides general information and self-help letter templates to help you prepare your own health insurance appeal. You choose what to include, fill in your own information, and decide whether and how to send it.

The Service is not legal advice, medical advice, or individualized professional advice. Using it does not create an attorney-client, nurse-patient, or other professional relationship. Although the Service was created by a registered nurse, no one reviews your situation or your letter. For advice about your specific case, contact a licensed attorney, your doctor, your state insurance department, or a consumer assistance program.

3. Your responsibilities

• You are responsible for the accuracy of any letter you send and for meeting all deadlines. Deadlines shown by the Service are general estimates based on federal rules. The deadline printed on your denial letter or in your plan documents controls.
• Plan rules, state laws, and federal rules change. We try to keep information current but cannot guarantee it is complete or up to date.
• You must be at least 18 years old, or have a parent or guardian's permission, to use the Service.

4. No guarantee of results

We do not guarantee that any appeal will be approved or that any claim will be paid. Outcomes depend on your plan, your medical situation, and decisions made by your insurer and reviewers.

5. No affiliation

MedAppealsRN is not affiliated with, endorsed by, or acting on behalf of any health plan, insurer, employer, or government agency. Insurer names appear only as examples to help you identify your type of coverage.

6. Purchases

The Appeal Kit is a digital product sold for $10 (US dollars). Payments are processed by Stripe. After payment you are sent to a download page. If a file will not download or open, contact us within 14 days at [CONTACT EMAIL] with your receipt, and we will fix the problem or refund your purchase. [CONFIRM REFUND POLICY.]

7. License

When you buy the Appeal Kit, you receive a personal, non-transferable license to use it for your own appeals or to help a family member with theirs. You may not resell, share publicly, or redistribute the files. The watermark must not be removed. All content on the Service is owned by us or our licensors.

8. Contributions

Support payments are voluntary contributions toward operating costs. They do not purchase any product or service, are not refundable except as required by law, and are not tax-deductible charitable donations. MedAppealsRN is not a nonprofit organization.

9. Limitation of liability

The Service is provided "as is" without warranties of any kind. To the fullest extent allowed by law, we are not liable for any indirect, incidental, or consequential damages, or for any loss arising from a denied claim, missed deadline, or reliance on the Service. Our total liability for any claim is limited to the amount you paid us in the 12 months before the claim.

10. Changes and governing law

We may update these Terms by posting a new version on this page. These Terms are governed by the laws of the District of Columbia, without regard to conflict-of-law rules.
